Job Summary:

Under the direction of the AFHT manager, the RAAM Clinic ALC Registered Practical Nurse assists with the daily operations of the RAAM Clinic Program, along with providing Patient Navigation support to community patients with a focus on reducing ALC patient designations within the hospital. This position supports individuals and families in the community, working collaboratively within an interdisciplinary care team to promote continuity of care, recovery and improved health outcomes. They also provide best practice assessments, treatment, navigation and support to individuals and families with substance use and/or concurrent disorders at the rapid access addiction medication (RAAM) clinic.

Requirements:

  1. Graduate from an approved School of Nursing
  2. Current certificate of Registration in good standing with the College of Nursing of Ontario
  3. Current certification in BCLS and Non-violent crisis intervention
  4. Knowledge of, and adherence to the Hospital’s Code of Conduct.
  5. Experience in community nursing, mental health or addiction services preferred
  6. Basic computer skills
  7. Successful completion of the Canadian Nurses’ Association specialty program in Psychiatric/mental Health Nursing is preferred.
  8. Current certification in buprenorphine training is preferred
  9. Current certification in Harm Reduction or Addictions is preferred
