Job Summary:
Under the direction and general supervision of the Long-Term Care Manager, The RPN / RAI Coordinator is accountable to provide quality nursing care to stable patients and residents whose outcomes are predictable while practicing within their scope as defined by the College of Nurses of Ontario and policies of the Hospital. The RPN / RAI Coordinator is also responsible for leading the RAI process, providing support and training to staff related to RAI, facilitating the completion of resident assessments, Resident Assessment Protocols (RAPs), and care plans of the long-term care residents and ensuring that all RAI related assessments are completed with accuracy within the allotted timelines and submitted and accepted to CIHI as per the CCRS date and submission timelines outlined by the Ministry of Long-Term Care (MLTC).
Requirements:
- Current registration as a Registered Practical Nurse with the College of Nurses of Ontario.
- Current BCLS.
- Basic computer skills.
- Credentialed MDS Resident Assessment Coordinator with successful completion of RAI-MDS Certification or willingness to obtain RAI Credentialing upon hire
- Demonstrated knowledge and work experience in the RAI-MDS assessment process.
- Comprehensive clinical background and knowledge in long-term care (minimum 2 years’ experience) and thorough knowledge of roles / responsibilities of other health care team members.
- Mandatory Covid-19 Vaccination.
